Every year, all grades (K-12) must review/update the required forms for the upcoming school year. This process is necessary for verifying your child's contact information, medical information, as well as signing off on the annual permission and acknowledgement forms for school district policies.
All forms should be submitted before students return to school on August 21, 2025.
Please follow these instructions:
Log into your PowerSchool Parent Account
Click on Forms in the left Navigation Bar
Click on the General Forms tab and complete the required 25-26 Required forms for the start of the school year. Review and complete forms, sign and date electronically. Each set of forms is unique to your students' building and grade level. You must review and complete a set of forms for each student.
Once the form is completed, click SUBMIT. A green check mark will appear next to the form as it is completed
You only need to complete the forms under the “****25-26 Required Forms for the start of School Year - TO BE COMPLETED BY PARENT/GUARDIAN”.
